Please be aware of the following policies.
We will verify your insurance 48 business hours prior to your appointment. We are happy to extend a 15% courtesy discount at the time of service if your vision insurance is unavailable or out of network. After payment, we will provide itemized billing receipts.
Copays are due at the time of service – we will bill your insurance out of courtesy to you, but it is your responsibility to be sure you are covered for this visit.
Cancel or reschedule requests must be received at least 2 full business days (not including weekends or holidays) before your appointment. To do so please reach out to us in the office, as this feature is not yet available online.
